Select the vertex and match it with the reference image. If you want to go to wireframe mode simply press F3 in your keyboard and if you want to you want to see both wireframe and shaded mode simply press F4. Now let us apply turbosmooth and check the showcage option, as shown below.

Now let us move up the joint part and then add 1-1 segment parallel to the place where the joint appear. Make it hexagonal shape. To add smoothness see below image.

Now let us add one more segment which would define the nail area as shown below.
