Activate the viewport on which you want to place the reference image and then go to Views menu > View Background or you can just press Alt+B key to open Viewport Background dialog box. In the Background Sources group, click the files button, here you are prompted with Select Background Image dialog box. Use the dialog to browse through and select the image file which we will be using as reference image for this tutorial. Click ok, that is it we have mapped the image to the viewport, you can see the Viewport Background dialog in the below image.

Now let us create a simple plane and then convert it to edit poly. Click Alt+X to switch to X-ray mode. Now let us move the vertex and give it basic shape as shown in the below image.

Let us extrude edges and giving it a basic shape of an ear as shown below.
